Athletic Program at Woodland Middle School
Our Intramural and Competitive programs are open to 6th, 7th and 8th grade girls and boys! Please make sure you complete the electronic Athletic Packet prior to the start of the activity to assure participation!! Athletic packets are good for the sports that you choose for the 2024-2025 school year and physicals are good for 1 year from the date of the exam.   

All students must have active health insurance which can be purchased through the link below.

• Athletic Forms -

  Athletic packets will be submitted electronically for the 2024-2025 school year.  I have attached forms that will need to be printed and uploaded when enrolling your child for competitive sports: a sports physical and a hold harmless form. You will also need to upload a copy of an active health insurance card.

  * Click ALL sports of interest when you enroll, otherwise you will have to return to the site again to add each sport.
